How to get it right: dealing with work on or around a party wall

It's now 20 years since the Party Wall etc. Act came into force. It applies throughout England and Wales and provides a framework for preventing or resolving disputes in relation to party walls, party structures, boundary walls and excavations near neighbouring buildings. So after 20 years are we getting it right?
Remedial work to existing buildings
An officer was called out when a builder uncovered substandard work on site. The proposal was to change a window opening into a new doorway. Having stripped away the plasterboard the builder was shocked to find an unclosed cavity, no wall ties and a lack of insulation. The opening also had a concrete lintel over the window spanning onto the internal blockwork. What was the best way to complete the work?
